new clutch slipping - what could be wrong?

well I just replaced the worn clutch and rear main seal of my 06 MK 2.8 TD with an exedy HD (I did most of the work and my mechanic the important bits) but now after a holiday of 4000 K's it is slipping again, not all the time but it does and usually in third, fourth and fifth gears.
The question is what could be wrong?

I take it the flywheel was machined.? clutch disc could be slightly glazed.
Ive had this sort of thing happen to be before and it turned out to be the new rear main leaking a little oil.
Odd I know it came good after about a week and a oil change.
To sort the clutch I slipped it a little on purpose while accelerating a couple of times and it came good.i tried this as I didn't want to pull box again. went on to do 100 thou with not an a single slippage after that

Thank you, no it turned out to be the pedal was too tight not letting the clutch engage properly.
